Job Description - SE Home-Visitor Supervisor

Description

SGA provides a wide range of social services in some of Chicago's most challenged and consistently underserved communities, and creates long-term and community-wide change.

MAJOR DUTIES AND RESPONSIBILITIES

  • Collaborate with Program Coordinator to strategize effective ways to build relationships within communities to recruit and help sustain caseloads.
  • In collaboration with the Program Coordinator, ensure that Home Visitors (HV) plan and implement developmentally appropriate weekly home-based activities for families/pregnant mothers within their assigned caseloads.
  • Coordinate recruitment, intakes and enrollment for all qualified children/families.
  • In collaboration with the program coordinators and supervisors create and maintain partnerships.
  • Encourage parent participation in PPC meetings, literacy activities, parent workshops, group connections/socializations, and other events or trainings.
  • Oversee the planning of the Group Connections and provide support to the Home Visitors, ensuring that all documentation and procedures are done and completed.
  • Support HVs to work jointly with parents to develop home visit and activity plans based on each child’s assessment and identified family needs.
  • Support HVs to ensure that PAT planning guides and personal visits records are being completed
  • In collaboration with program Coordinator, support and monitor the data systems that the program uses and support staff when needed
  • Support HVs to ensure that developmental screenings and family needs assessment are being completed.
  • Maintain and preserve confidentiality.
  • Support staff in the referral process based on the needs of the families.
  • Assist staff in maintaining 100% enrollment rate throughout the program year utilizing various strategies. 
  • Support and facilitate staff meetings when necessary.
  • When needed conduct home visit observations and Group connection Observations utilizing the PAT Observation tools and provide feedback.
  • Analyze CP/data system reports and provide analytical data to Program Coordinator and funder.
  • Participate in ongoing training and staff development provided by the agency, funder, Coordinators, home-visitor supervisors, consultants, workshops, and conferences.
  • Attend regularly scheduled team and agency staff meetings.
  • Responsible for the completion and submission of purchase orders and check requests when needed.
  • Assist supervisor with the hiring of staff.
  • Responsible for training, coaching and guiding new Home Visitors.
  • Help maintain the inventory of client incentives.
  • Provide leadership during Self-Assessment and Program’s reviews
  • Complete other duties as assigned.

Requirements

QUALIFICATIONS FOR THE JOB

Education:

  • Bachelor's in EC, Gateways Credential 

Experience:

  • Experience working with young children ages 0-3 years old and pregnant moms
  • Knowledge of Parents As Teachers, preferred.
  • Knowledge managing CP/data systems preferred
  • 1-3 years of supervising or coaching experience

Other:

  • Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals, and community groups. 
  • Must be able to make home visit observations in various locations surrounding the  Chicago areas.
  • Must be able to attend training/meetings in various locations  as other locations assigned by the supervisor
  • Ability to write routine reports and correspondence.  
  • Ability to speak effectively before groups of customers or employees or the organization.
  • Exercises discretion and independent judgment in executing tasks.
  • Bilingual (English & Spanish) required.
  • Must demonstrate the ability to establish helping relationships with families of varying educational, experiential, and socio-economic backgrounds and handle emergency and crisis situations.

KEY COMPETENCIES

  • Competent in Microsoft Office tools and inputting services in an online database.
  • Staff must receive training in education, nutrition, and other topics relevant to the position, and must plan and implement appropriate family/parent educational activities and function as a member of the program team.  
  • Schedule must be flexible and will include evenings and weekends.
  • Valid driver’s license, car insurance, and access to an automobile. 

PHYSICAL REQUIREMENTS

  • Must be able to remain stationary 50% of the time.
  • Must be able to frequently move inside the office to access file cabinets, office machinery, etc. 
  • Must be able to transport oneself to and from home visits, training, and meetings
  • Occasionally ascends/descends stairs.
  • Consistently positions oneself to assist children during activities.
  • Frequently moves desks, chairs, tables, easels, and children's toys. 
  • At times may be required to lift up to 25 lbs. for various classroom and event needs. 
  • Ability to move in outdoor weather conditions.
