Provides initial assessment of needs to providers of perinatal people.

Provides resources and referrals to providers.

Documents consultation encounters in a timely manner and in compliance with project requirements.

Maintains care coordination data to support program operations and reporting.

Maintains a good working relationship and ongoing communication with referral sources.

Oversee other resource and referral specialists for mental health access programs.

Manage the PROSPER website.

Promotes a positive working environment and customer relations.

Coordinates enrollment meetings with healthcare practice.

Promotes educational events to healthcare practices and providers.

Qualifications:

Minimum Qualifications:

Bachelor's degree in Psychology, Counseling, Social Work, Public Health, or a directly related field from an accredited institution.

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

Two (2) years of professional level experience working with mental health issues and disorders, specifically perinatal mood, anxiety, and/or substance use disorders

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:

Master's degree in Psychology, Counseling, Social Work, Public Health, or a directly related field from an accredited institution.

Spanish language proficiency.

Experience with REDCap or other data management program.

Experience with Wix for website building and management.

Experience with and/or training in perinatal and infant mental health.

Experience with and/or training in substance use disorder treatment.

Experience with crisis management or triage.

Experience with program development, evaluation, and/or process improvement.

Experience using Electronic Health Record and Patient Health Information management systems.

Knowledge, Skills, and Abilities:

Ability to communicate effectively, both in writing and orally.

Ability to establish and maintain effective working relationships with employees at all levels throughout the institution.

Outstanding customer service skills.

Demonstrated commitment and leadership ability to advance diversity and inclusion.

Knowledge of mental health issues and disorders, specifically perinatal mood, anxiety, and substance use disorders.

Knowledge of evidence-based treatments for mental health conditions, and awareness of resources in Colorado communities.

Comfortable with case management and coordination of care.

Ability to triage effectively based on expressed acuity by the health care provider about the consultation.

Efficient time management skills and ability to meet deadlines.

Knowledge of HIPAA regulations.

General knowledge of health care systems, payors and care navigation.

Comfortable working in high volume, fast paced office environment.

Proficient in Microsoft Office programs and ability to learn software programs.
