Over 12,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:
We are currently seeking an experienced Senior Case Manager to coordinate care for patients admitted and transferred to the Stacy Kirkpatrick House (SKH) Stay-in Bed Program from the Barbara McInnis House. The Stacy Kirkpatrick House is a step-down respite facility, welcoming patients previously recovering from surgeries, long-term treatments, and other ailments at the Barbara McInnis House. As the Senior Case Manager, you will be directly involved in managing the social environment of Stacy Kirkpatrick House and providing supportive patient-focused interventions.  You will also help coordinate patient activities, be the point person for volunteers at Stacy Kirkpatrick House, follow up on the discharge plans started by the Barbara McInnis House case managers and coordinate with the clinic nurse regarding medical stability and readiness for discharge.

Hours: Friday and Saturday 7pm-7:30am

Responsibilities
  • Work as part of a multidisciplinary team alongside the clinic nurse, provider, and program site director to help manage the milieu of the stay-in beds and address the needs of the patients.
  • Provide detailed shift reports of any pertinent patient or building issues to the senior case manager coming in on the next shift and keep the Director of SKH informed of all issues that arise.
  • Communicate with the Volunteer Coordinator regarding volunteers at SKH to run patient activities, act as the point person at SKH for the volunteers, and coordinate transportation to and from appointments in collaboration with the Manager of Clinical Support Services.
  • Provide resources to assist patients in breaking cycles of homelessness, substance use disorders, and/or mental illness; provide resources to secure appropriate discharge placement and, when possible, other appropriate alternatives to shelter living.
  • Facilitate transfers and discharges to and from the stay-in beds; greet new patients coming into SKH and help them to get settled in by reviewing rules and regulations; assist patients in making follow-up calls to secure a safe discharge plan such as to a treatment program or halfway house.
  • Alert housekeeping to patient discharges to ensure timely cleaning of the newly vacated bed space; in the absence of housekeeping staff, provide cleaning support in the event of a spill or other event; assist with linen stocking, clinic stocking, bed checks, meal set up and clean up as needed.
  • Report all discharge planning processes to the SKH Director and SKH Supervisor of Case Management to ensure clear communication regarding the discharge plan; build on the discharge plan from week to week and demonstrate this process in team discussions and notes.
  • Monitor the stay in beds off shift and assist with emergencies by calling security, EasCare, or 911 as appropriate; provide support to the nurse while they are providing nursing care.
Qualifications:
  • Previous case management or human service experience required preferably with 3-5 years of experience; prior experience working in a group home or housing program preferred; experience and Training in Motivational Interviewing.
  • CPR certified and trained in providing emergency services such as administering Narcan until an ambulance arrives, or willing to become certified and trained.
  • Excellent communication, documentation, and organizational abilities; team player; ability to be flexible, multi-task, prioritize responsibilities, work in a fast-paced environment, and handle stress in a positive manner.               
  • Demonstrate interest in working with under-served populations; willingness to assist clinical staff as they assess and treat patients.
Compensation and Benefits:
  • The compensation starts at $22.07 and increases with years of experience
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional annual compensation of $7,500 for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
