This is a full-time pharmacist position within our Anticoagulation and Pharmacotherapy Clinics, which are located across Snohomish County. The pharmacist will be working directly with patients in the clinic to manage multiple conditions. Our current services include Anticoagulation, Dermatology, Diabetes, HCV, HIV/PrEP, GI, LAIAs, Lipids, and Osteoporosis. The successful candidate may be assigned to work to one or multiple of these services, depending on their background, experience, and clinic needs.
The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
