General Responsibilities:

1.   Handles a wide array of administrative tasks to ensure compliance with agency/programmatic policies and procedures. 
2. Conducts wellness check calls to clients enrolled in the home-delivered meals program to: Evaluate their overall well-being, including physical and emotional health, Verify they are receiving the meals and assess if there are any issues related to meal delivery, Assess safety of clients and follow up accordingly, In the event of a crisis (medical emergency, mental health distress) provide immediate support by following established protocols. 
3. Completes telephonic surveys with clients to assess satisfaction with the program and to evaluate effectiveness of services.
4. Communicates with food vendor(s) regarding start and stop dates for service. 
5. Communicates with other Catholic Charities staff and Managed Care Organization staff regarding client’s services and client status. 
6. Documentation of all client contacts, client status, follow ups and outcome in case recording system. 
7. Data entry into program and state required software/databases. 
8. Other duties as assigned within the guidelines of this position.

Essential Functions:

1. Works collaboratively with Catholic Charities staff and service providers to ensure implementation of nutrition services to clients in accordance with funder guidelines.
2. Receives and processes referrals from the Community Dining Site Coordinators, Care Coordination Unit, Nutrition Program Staff, and Managed Care Organizations.
3.  Informs Program Manager of any challenges, problems, or concerns in a timely manner.
4. Maintains up to date and accurate client files and databases, as required by Agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory licensing or accrediting bodies in the course of      performing job expectations. 
5. Assists with Program Quality Improvement and some Administrative support functions, including reviewing files, processing intakes, processing terminations, and electronic case recordings. 
6. Mails correspondence to clients and processes returned mail as needed for the program.  
7. Attends staff meetings, conferences, and training programs as recommended by supervisor.
8. Occasional coverage of the front desk receptionist duties in the absence of the Office Coordinator and fill in coverage.

Requirements

1. A high school diploma or equivalent is required for this position; or an Associate’s degree is preferred with preferences in areas of human services, social work, psychology, sociology or related field.  All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
2. Minimum of one (1) year of experience working with persons from diverse social backgrounds is preferred.
3. Minimum of one (1) year of experience working with older adults is preferred. 
4. Ability to relate well to persons with disabilities and older adults, respecting their privacy, dignity, and right to self-determination. 
5. Should be caring, compassionate, and able to connect with older adults. 
6. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams. Must have knowledge of the basic functions and use of Excel for all data tracking related to the Nutrition Program.
7. Strong verbal and written communications skills.
8. Strong organizational and time management skills.
9. Passage of complete background clearance, and any other required testing or clearances.
