Job Title: Service Coordinator/Home Services\n\nUnit: Help Me Grow\n\nShift: Days-FT (8:00 a.m.-4:30 p.m.)\n\nFacility: Miami Valley Hospital\n\nThe Service Coordinator functions in a multifaceted role that demonstrates professionalism and promotes excellence in service to eligible families. The Service Coordinator/Home Visitor is the single point of contact in helping parents to obtain necessary services and assistance. The functions include integrating all direct services with those program services already provided by other public or private sources. Home visits are made with the caseload of children and families using the prescribed curriculum. The Service Coordinator/Home Visitor conducts assessments, assists in child find activities, collaborates with families and agencies in developing, implementing and monitoring the Individualized family plan, updates indicated data base regularly, and facilitates transition to preschool services.\n\nThe Service Coordinator/Home Visitor reports to the Home Visit Coordinator for Ongoing Services. The SC/HV is accountable for compliance with all applicable regulatory standards, policies, procedures, and protocols of the Human Resources Department and any funding sources for the work of the department. The SC/HV will comply with Indicated data base and data entry.\n\n \n\nEducation:Minimum Level of Education Required: Bachelor\u0027s degree \n \nAdditional requirements: \n\uf0a7 Type of degree: Bachelor\u0027s degree \n\uf0a7 Area of study or major: Social Work \n\uf0a7 Preferred educational qualifications: LSW \n\uf0a7 Position specific testing requirement: Must undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio central registry on child abuse and neglect. \n\nLicensure/Certification/Registration: Licensed Social Worker\n\nExperience: \n\nMinimum Level of Experience Required: 1- 3 years of job-related experience \nPrior job title or occupational experience: Minimum two years\u2019 experience with young children and their families. Prefer home-visiting program experience. Demonstrates a thorough knowledge of and the ability to work with community resources appropriate to the needs of at-risk populations. \nPreferred experience: At least 1 year working with pregnant women, infants, and children. \nOther experience requirements: Complete orientation per the Ohio Department of Developmental Disabilities guidelines. \n\nKnowledge/Skills\n\n\u2022 Must possess and display effective verbal, written and interpersonal communication skills. \n\u2022 Organized and self-directed. \n\u2022 Valid driver\u2019s license and insured safe automobile. \n\u2022 Non-smoker\n\n \n
