• Support clients and families receiving I/DD services to navigate their care and achieve their goals

• Coordinate services, complete supervisory home visits, and attend service planning meetings

• Provide guidance on Medicaid waivers (CES, SLS, and DD) and help families understand available resources

• Collaborate with nurses to ensure health and safety needs are met in service planning

• Work closely with Direct Support Providers (DSPs), scheduling training, coordinating meetings, and ensuring quality care

• Attend community events and build relationships with agencies to strengthen support networks

✔ Experience in case management, client coordination, or customer service
✔ Background in intellectual and/or developmental disability services (preferred)
✔ Knowledge of Medicaid waivers and caregiving is a plus!
✔ Bilingual (Spanish/English) is preferred, but not required
✔ A problem-solver and strong communicator who is passionate about improving lives
