The Service Coordinator is responsible for providing support to people receiving services from Trivium Life Services. This role is responsible for ensuring each person served is provided the opportunity to enhance their life through the process of a person-centered plan. The Service Coordinator will report directly to the Service Coordinator Manager or the Market Director.

While performing the responsibilities of the Service Coordinator, the employee is required to talk and hear.? The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard.? The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl.? Vision abilities required by this job include close and distant vision.
Working Conditions
Working conditions are normal for an office environment.? The noise level in the work environment is usually quiet to moderate.? Work will require weekend and/or evening work as needed.? Occasional conditions will vary as it will be necessary to be in various locations, homes of persons served or their families and funding agencies.? The employee must be able to travel between locations, and occasionally attend seminars which may require an overnight stay.
