THE ORGANIZATION
Alta California Regional Center (ACRC) is a non-profit agency under contract to the State of California and for 50 years has provided the coordination of services to individuals with developmental disabilities. ACRC employs approximately 700 persons working out of eight offices in a ten-county area. ACRC is one of 21 Regional Centers in California.
COMPENSATION
The below is dependent upon experience and education.
Service Coordinator I: $21.01 - $26.78/hr.
Service Coordinator IIIB: $24.63 - $31.43/hr. (must possess a master's degree and 1-year social work experience)
Service Coordinator IIIA: $27.19 - $34.71/hr. (must a possess a master's degree and 2-years post-masters experience)
The actual amount offered within this range will depend on a variety of factors including skills and abilities, education, experience, and other relevant job-related factors. It is not typical for an individual to be hired at or near the top of the range for the role.
THE POSITION AND JOB SUMMARY
The Service Coordinator for Acute Care Coordination (SC-ACC) position is located in the Sacramento office and will travel within ACRC's 10 county catchment area, and as needed, outside of ACRC's catchment area to support Service Coordinators. The SC-ACC position may carry up to five clients and will report directly to an Associate Director of Client Services. The SC-ACC will serve as facility liaison to one Incidental Medical Services residential facility. The SC-ACC is responsible for providing community outreach, training and serves as the point of contact to local hospitals and nursing facilities regarding ACRC clients in-care. The SC-ACC will provide consultation, training and support to Service Coordinators related to discharge planning, use of generic resources and placement opportunities as requested. The SC-ACC will lead HEART Committee staffing's and act as liaison between Service Coordinators, hospitals and nursing facilities. The SC-ACC will oversee gathering data from the HEART Committee to analyze why our clients needed immediate acute care and have no viable discharge option. Additionally, the SC-ACC will join internal committees as necessary to provide consultation and/or support. The SC-ACC will provide a monthly summary of protracted hospital stays and perform other duties as necessary.

JOB REQUIREMENTS AND QUALIFICATIONS
Applicants must possess a Master's or a Bachelor's degree in Social Work or a related field; demonstrate excellent written, verbal, and interpersonal communications skills; have strong time management and organization skills; and possess a valid California driver license and automobile liability insurance.
