Service Coordinator - Mentor and Case Manager

Are you passionate about helping youth overcome challenges and achieve their full potential? CGC (Child Guidance Center) is seeking a Service Coordinator to join our Community Action Team (CAT)—a high-impact program dedicated to supporting children and young adults (ages 6–21) with significant mental health needs.

As a Service Coordinator, you’ll provide case management and mentoring services to youth and families, helping reduce out-of-home placements and build long-term stability.

What You’ll Do:

• Deliver case management and mentoring in homes, schools, and community settings
• Partner with families and teams to create and implement service plans
• Coordinate care, link families to resources, and support crisis stabilization
• Assist with job readiness, social skills, and safety planning
• Document services in an electronic health record system (EHR)
• Participate in team supervision, peer review, and quality assurance

Compensation and Benefits:

• Starting hourly rate ranges from $17.78 to $18.75

Education and Experience Requirements:

• Bachelor’s degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related field which includes the study of human behavior and development.
• A minimum of one year of experience working with children or adolescents with serious emotional disturbances and behavioral needs.
• Current FL driver’s license and good driving record plus reliable transportation with automobile liability insurance is also required.

Preferred Education and Experience:

• Certified in Targeted Case Manager is preferred
• Trained in wraparound, family systems, crisis intervention, teenager or young adult suicide prevention and trauma-informed care
• Three (3) plus years of experience working directly with crisis level adolescent is preferred.
