Service Coordinators provide direct supports and network linkages to children, families and the Department with the goal of coordinating care and monitoring family-based services and ongoing formal and informal community supports.

What You Will Be Doing to Make a Difference...

• Develop and maintain collaborative relationships with state agencies, residential programs, group homes, service providers, and community organizations at the area, regional, and state levels
• Develop community partnerships with formal and informal community resources thus enabling access to expanded, ongoing community-based services for the children and families of DCF
• Develop a Network and partnership of Preferred Providers who support the creation and implementation of a service delivery system that is integrated, collaborative, flexible, and responsive to the unique needs of the children and families being served
• Identify and actively support the development of new and necessary service models supporting permanency and community tenure
• Ability to motivate others and manage conflict
• Driving is required

Schedule is Mon-Fri 8:45am-5pm.

Requirements

• Bachelor’s degree in Human Service field required
• Minimum of 3 years of experience working in child and family treatment (assessment, treatment planning, outcome measurement, with systemic, behavioral, and developmental focus)
• Capacity to work within greater Coastal or Plymouth Area and larger South Shore Area
