Job Title: Services Coordinator Lead (two positions available – must be a current employee with Developmental Disabilities Services)

Salary: $28.05 - $35.47 per hour

Developmental Disabilities Services is seeking two compassionate and dedicated Service Coordinator Leads to provide targeted case management services to individuals experiencing Intellectual/Developmental Disabilities (I/DD). In this rewarding role, you will have the opportunity to identify individual needs, develop plans to access services and resources and support the overall effectiveness of the Services Coordination team.

The Ideal Candidate
As a Services Coordinator Lead, you will play an integral part in case management practices and coordination. This position requires independent work, sound judgment, strong communication skills and occasional guidance for new situations. We are looking for an individual that works well in a collaborative team environment and approaches challenges with a solution-oriented mindset.

Key Responsibilities

Develop and periodically revise Individual Support Plans (ISPs) or Annual Plans based on assessments.
Assist individuals and families in implementing support systems.
Utilize choice advising for service, provider, and setting options.
Monitor effectiveness and Medicaid standards of plans and authorized expenditures.
Deliver person-centered services, promoting self-determination.
Coordinate with various agencies for comprehensive service delivery.
Maintain compliance with relevant laws, rules, and policies.
Independently manage a caseload and document interactions accurately.
Participate in transition planning and support complex needs cases.
Support the Services Coordination team, review case management tools, and provide peer support.
Act as a point of contact for choice counseling and facilitate team discussions.
Report service delivery gaps and promote consistent case management practices.

Qualifications

Education:
- Bachelor's degree in behavioral science, social science, or related field and one year of related experience.
- Or, bachelor’s degree in any field and two years of related experience.
- Or, associate’s degree in behavioral or social science and three years of related experience.
- Or, four years of related experience.
Knowledge:
- Public service system for developmental disability services in Oregon.
- Klamath County community agencies and resources.
- Confidentiality and mandatory reporting requirements.
- Federal, state, and county laws governing ID/DD services.
Skills:
- Teamwork, effective communication, and customer service.
- Working with individuals with intellectual or developmental disabilities.
- Managing information and maintaining confidentiality.
- Using Microsoft Office and specialized software.
Abilities:
- Establish professional boundaries and maintain confidentiality.
- Promote the mission and core values of KCDDS.
- Develop realistic service plans and manage complex projects.
- Provide constructive feedback and recommend solutions.
- Handle sensitive information and act professionally.
