SH 399 SH RIVERTON PHYS PRAC - Travel: LPN Clinic Days

Responsible for providing direct nursing care to patients under the direction of a physician or health care provider. Assesses, plans and evaluates patient care needs. Monitors, records, and communicates patient conditions as appropriate utilizing computerized documentationsystems. Assists physician during examination, treatment, and procedures. Carries out physician's orders.

Facility Requirements:
Must have BLS certification, must be attached to submission packet (Must be through the American Heart Association); RQI certifications will not be accepted
Must have WY LPN license
Must have 3 years recent LPN clinic/physician practice experience
Shifts will be Monday - Friday 8a-5p
