We’re looking for a Physical Therapist who not only delivers excellent care—but also brings these values to life.

‍⚕️ Position Summary

As a Home Health Physical Therapist, you will provide personalized, one-on-one care to patients in their homes—helping them regain mobility, independence, and quality of life.

This is a field-based role offering flexibility, autonomy, and the opportunity to build meaningful patient relationships.

✅ Key Responsibilities

Patient Care

• Evaluate and treat patients recovering from injury, illness, or surgery
• Develop and implement individualized plan of care
• Improve patient mobility, strength, balance, and functional independence
• Provide interventions focused on fall prevention and safety

Care Coordination

• Collaborate with physicians, nurses, and interdisciplinary team members
• Adjust treatment plans based on patient progress
• Participate in case conferences and care planning

Patient & Caregiver Education

• Teach patients and families exercises, safety strategies, and mobility techniques
• Empower patients to manage their conditions at home

Documentation & Compliance

• Complete accurate and timely clinical documentation (OASIS experience preferred)
• Follow all Medicare home health regulations and Oregon state requirements

Quality & Outcomes

• Support improved patient outcomes and satisfaction
• Reduce rehospitalizations and promote safe recovery at home

Qualifications

• Licensed Physical Therapist (PT) in Oregon (or eligible)
• Graduate of an accredited Physical Therapy program
• Valid driver’s license and reliable transportation
• Ability to work independently in a field-based setting

Preferred

• Prior home health experience
• Familiarity with OASIS and Medicare documentation
• Experience serving rural or geographically spread communities

Compensation & Benefits (Oregon Coast Market)

Compensation

• Salary Range: $55 – $65 Hourly (DOE)
• Sign On Bonus: $10,000
