Job Description

We are seeking a compassionate Physical Therapist to join a skilled nursing facility in Potosi, MO, on a travel contract. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapy services to improve mobility, restore function, and enhance independence. The ideal candidate is patient-focused, adaptable, and experienced in geriatric rehabilitation within long-term or skilled nursing care.

Key Responsibilities

• Conduct comprehensive patient evaluations to determine functional abilities, therapy needs, and goals.


• Develop and implement personalized treatment plans in coordination with physicians and care teams.


• Deliver therapeutic interventions including strengthening, balance training, gait training, and mobility exercises.


• Educate residents, families, and caregivers on therapy objectives, safety techniques, and use of assistive devices.


• Document evaluations, treatment sessions, and outcomes accurately in compliance with facility and regulatory standards.


• Collaborate with nursing and therapy staff to ensure coordinated, patient-centered care.


• Promote resident safety, independence, and quality of life through evidence-based therapy practices.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.


• Current and valid Physical Therapist license in Missouri (or compact license).


• Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.


• Current certification in Basic Life Support.


• COVID-19 vaccine and recent tuberculosis test.


• Strong assessment, documentation, and communication skills.


• Compassionate, patient-centered approach with experience supporting geriatric populations.
