Job Description - Skilled Nursing Social Services Assistant



Full-time


Description

This position plays a critical role in providing support, advocacy, and resources to residents and their families.  This role will assess residents’ psychosocial needs, develop care plans, and coordinate services to enhance their overall well-being and quality of life. 

Primary Responsibilities and Duties: 

  • Conduct comprehensive psychosocial assessments of residents upon admission and on an ongoing basis to identify their social, emotional, and environmental needs. 
  • Collaborate with residents, families, caregivers, and other team members to develop individualized care plans that address residents’ psychosocial needs, preferences and goals.  
  • Provide counseling, support, and crisis intervention services to residents and families experience emotional distress, adjustment issues, or significant life changes. 
  • Advocate for residents’ rights and preferences, ensuring they are respected, protected, and upheld in all aspects of care and decision-making. 
  • Assist residents and families in accessing community resources, financial assistance programs, and other support services to address social, economic, and environmental challenges. 
  • Coordinate discharge planning and transitional care services to facilitate smooth transitions for residents returning home or transferring to other care settings.  
  • Collaborate with external agencies, healthcare providers, and community organizations to coordinate and enhance serves for residents and families.  
  • Maintain accurate and confidential documentation of assessments, care plans, interventions, and progress notes in compliance with regulatory requirements. 
  • Participate in team meetings, case conferences, and care planning meetings to contribute insights, coordinate services, and ensure continuity of care. 
  • Provide education and training to staff members on social services issues, resident rights, and psychosocial support strategies. 
  • Perform all other duties assigned.  

Schedule: 

  • Full-Time
  • Monday-Friday
  • 8 Hour Shifts

Requirements

Job Qualifications: 

  • Current state licensure or eligibility for licensure in social work preferred.
  • Minimum of 1 years’ experience in social services or related field preferred, especially in a healthcare or long-term setting.  
  • Knowledge of psychosocial assessment techniques, counseling theories, and therapeutic intervention for older adults. 
  • Familiarity with community resources, government benefits programs, and social services agencies. 
  • Ability to build rapport with residents through a compassionate and caring demeanor.
  • Excellent interpersonal and communication skills, with the ability to collaborate effectively with residents, families, and team members. 
  • Commitment to providing high-quality, resident-centered care with empathy and compassion. 

Physical Requirements:

  • Work performed in a medical setting due to the need to work directly with residents, their families, and other team members. 
  • Must be able to sit, stand, bend, reach, talk, hear, stoop, kneel, crouch, stretch, and twist.
  • Auditory ability to communicate effectively with residents, families and team members  
  • Physical stamina to handle the demands of a fast-paced and dynamic healthcare environment. 
  • Tolerance for exposure to bodily fluids, medical equipment, and infectious diseases. 
  • Ability to work in various environmental conditions, including exposure to noise, temperature variations, and odors. 
  • Required vision includes close, vision, distance, peripheral, and the ability to adjust focus. 
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations. 
  • Stamina to work full shifts and perform repetitive tasks throughout the day, including walking or standing for extended periods.  

Salary Description

$22-$24 Per Hour
