Riverwood Center is located in beautiful Southwest Michigan with staff at seven sites across Berrien County. We are accredited by the Commission on Accreditation of Rehabilitation Facilities and a member of Southwest Michigan Behavioral Health, Michigan Association of Community Mental Health Boards, and the National Council for Behavioral Health.

Responsibilities:
·          Provides case management services in the community to consumers with severe mental illness
           and their families. 
·          Assists in planning, linking, advocacy, coordination and monitoring; to assist consumers in 
           gaining access to health services, financial assistance, employment, education, social services
           and natural supports.
·          Assesses needs, develops and monitors Person Centered Plans and documents treatment
           in an accurate and timely manner.
·          May provide coordination of care between the criminal justice system and consumers
           with mental illness that participate in the Berrien County Mental Health Court and/or the
           prisoner re-entry initiative.
 
Qualifications:     
·          Bachelor Degree in an appropriate mental health discipline.
·          One or more years’ experience providing treatment to individual’s mental illness. 
·          Excellent computer and interpersonal skills, punctuality, ability to organize and manage 
           several projects/tasks simultaneously, and work independently. 
·          Must have reliable means of transportation for community outings and transporting consumers.
