About the Role:

The client is currently seeking an experienced In-Person Nurse Practitioner to join the team at Hamilton Park Nursing and Rehabilitation Center in Brooklyn. This is a full-time, W-2 position ideal for a clinically skilled NP with a strong background in SNF, home care, or hospital-based care. The ideal candidate will be confident in autonomous practice, compassionate, and well-versed in managing chronic and acute conditions in older adults.

Key Responsibilities:

• Provide in-person primary and acute care to SNF residents.
• Perform routine evaluations and manage chronic conditions.
• Conduct assessments, develop treatment plans, and document all encounters.
• Collaborate with interdisciplinary facility staff including physicians, nurses, and social workers.
• Ensure compliance with regulatory standards and documentation requirements.
• Educate patients and families on care plans, medications, and wellness strategies.
• Utilize evidence-based practices to improve patient outcomes.

Qualifications:

• Active Nurse Practitioner license in New York State (required)
• Minimum of 3 years of clinical experience in SNFs, home care, or hospital settings
• Strong clinical skills and the ability to work independently
• Proficiency in medical documentation and EHR systems
• Excellent communication and interpersonal skills
• Passion for elder care and value-based clinical practice
