SNF Transporter

Job Summary

The SNF Transporter is responsible for the safe, timely, and dignified transport of residents within the facility and to and from off-site appointments, including dialysis, medical appointments, and hospital visits. This role requires safe vehicle operation, proper resident transfer technique, and close coordination with nursing staff to ensure resident safety and comfort throughout every trip.

Essential Duties & Responsibilities

? Safely transport residents via wheelchair, gurney, or facility vehicle to and from appointments, procedures, and activities.

? Assist residents with transfers in and out of vehicles, wheelchairs, and beds using proper body mechanics and facility lift/transfer protocols.

? Verify resident identification and transport orders prior to each trip.

? Communicate with nursing staff regarding resident status, special needs, or precautions before transport.

? Ensure resident safety restraints/seatbelts are properly used during all vehicle transport.

? Maintain accurate transport logs, including departure/return times, destination, and any incidents.

? Report any change in resident condition observed during transport to nursing staff immediately.

? Maintain cleanliness and proper functioning of transport vehicles and equipment.

? Assist with loading and unloading medical equipment as needed (oxygen tanks, wheelchairs, etc.).

? Follow all infection control precautions during transport.

? Maintain resident confidentiality in accordance with HIPAA requirements.

? Perform other related duties as assigned.

Qualifications

Required

? High school diploma or equivalent.

? Valid California driver's license with a clean DMV record.

? Ability to safely operate facility transport vehicles.

? Basic understanding of resident safety and proper body mechanics.

? Strong communication skills and ability to work as part of a care team.

Preferred

? Active Certified Nursing Assistant (CNA) license.

? Prior experience in a skilled nursing facility, hospital, or medical transport setting.

? Current CPR/BLS certification.

Physical Requirements

? Ability to safely lift and assist with resident transfers, with or without assistance, per facility lift policy.

? Ability to push wheelchairs and stand or walk for extended periods.

? Ability to bend, stoop, and reach as needed to assist with resident transfers and equipment.

? Ability to safely enter and exit a vehicle repeatedly throughout a shift.

Work Environment

? Work is performed within the skilled nursing facility and in community settings, including medical offices, hospitals, and dialysis centers.

? Exposure to varying weather conditions during vehicle transport.

? Potential exposure to infectious materials; personal protective equipment (PPE) provided per facility protocol.

Other Requirements

? Must maintain a clean DMV record throughout employment; DMV record is subject to periodic re-verification per facility policy.

? Must complete facility orientation and any required training prior to independently transporting residents.

? Must pass a background check and health screening per facility and regulatory requirements.
