Social Care Network Care Navigator

Description

The Social Care Network Care Manager works independently or as part of a larger team to provide screenings, navigation of services and case management for eligible Medicaid adults and children as they relate to their social determinants of health needs.

Social Care Network Care managers will provide short term or ongoing linkages to other local, state, and federal benefits and programs. Specific responsibilities will include case management, outreach, referral management and education, including linkages to other state and federal benefit programs, benefit program application assistance, and benefit program application fees. Connection to clinical case management. Connection to employment, education, childcare, and interpersonal violence resources. Consistent monitoring outcomes of linkages and adjusting as needed. The Care Manager guides program enrollees and their caretakers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.

Requirements

• High School Diploma
• 2 years’ relevant experience in serving children, adults and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services. 
• Experience coordinating and participating in team settings. 
• Ability to deliver family-driven youth guided services. 
• Spanish Speaking Preferred.
