Job Description - Social Care Network Navigator

Job Overview: Responsible for identifying and addressing health-related social needs of Medicaid members within Chautauqua County by screening individuals, connecting them to appropriate community-based services like housing, but not limited to food assistance, transportation, and ensuring seamless access to these services through a network of providers, all while maintaining detailed documentation.



Qualifications:


Associates degree required with knowledge in the healthcare field preferred. Requires excellent organizational, communication skills with attention to detail. Flexibility and the ability to work well with a variety of groups and individuals. Experience working in a culturally diverse community setting. Sensitivity to cultural differences. Valid driver’s license with a good driving record and insurance. Requires some county-wide travel with some evening and/or weekend hours. Knowledge in Substance Use Prevention and Bi-Lingual preferred but not required.


 


Required Skills:



  • Knowledge of Microsoft office

  • Strong understanding of social determinants of health and their impact on healthcare outcomes

  • Experience in case management, outreach, and community engagement with vulnerable populations 

  • Familiarity with Medicaid eligibility guidelines and local social service programs 

  • Excellent communication and interpersonal skills to build rapport with clients and community partners 

  • Ability to navigate complex systems and effectively manage multiple priorities 

  • Proficiency in data entry and utilization of electronic health records (EHR) systems 


 


Description of Duties:



  • Conduct comprehensive screenings with Medicaid members to identify health-related social needs like housing instability, food insecurity, transportation barriers, and lack of access to healthcare.

  • Utilize knowledge of local community resources to connect individuals with appropriate social service providers based on their assessed needs.

  • Assist individuals in navigating the application process for social services, including completing necessary paperwork and following up on referrals.

  • Advocate for clients' needs with service providers and other stakeholders to ensure access to appropriate services.

  • Maintain accurate documentation of client interactions, referrals, and outcomes within the Social Care Network platform.

  • Participate in outreach activities to raise awareness about the Social Care Network and available services within the community.

  • Other duties assigned by the Executive Director who supervises this position.
