Position Description:
The Social Service Worker provides professional social services, including, but not limited to, psychosocial assessment, patient advocacy, discharge planning, resource management, crisis intervention, community education, and supportive counseling, designed to meet the varying psychosocial needs of patients and their families as these issues relate to the current hospitalization or subsequent transition from the acute hospital setting. This positions provides professional social services to high-risk outpatients, including those in the Cancer Care Resource Center, Diabetes Management Center, Infusion Center, Outpatient Imaging Center, and Outpatient Pulmonary Rehabilitation.
Qualifications
1. Bachelor’s Degree in Family and Consumer Science, Social Work, Sociology, Psychology, or a related field.
2. 3-5 years of clinical experience preferred.
3. Basic computer skills. Experience in Microsoft Word and Excel beneficial.
4. Effective verbal and written communication skills necessary.
5. Thorough assessment skills.
6. Demonstrated Leadership ability.
7. Must possess excellent time-management skills and be able to effectively prioritize work duties.
