Social Services Assistant

We need a full-time Social Services Assistant to join our team at Hillspring Skilled Nursing and Rehabilitation in Springboro, OH.

You’ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained.

$16.50 - $20 Per Hour

You’ll Be Responsible For:

• Assisting with gathering information from residents and family members

• Assisting with the completion of the Integrated Personal Services Assessment and MDS

• Assisting with the discharge planning process by arranging community resources, alternative placement, etc.

• Attending Care Conferences as directed by the LSW

• Assisting LSW with any other case management tasks deemed necessary to the Department.

• Fulfilling basic documentation requirements for social services.

• Assisting (after training) in the BIMS and the PHQ-9

You’ll Need To Have:

• Excellent communication skills

• A High School Diploma and/or GED

• Healthcare industry, social work, or other related experience

• The ability to multi-task in a fast-paced, high volume environment

• Exceptional organization skills with strong attention to detail

• A personable and professional demeanor

• Computer proficiency in Excel, Word, Power Point, and Outlook
