SOCIAL SERVICES COORDINATOR – Full-Time - Generations at Regency, Niles, IL

Generations at Regency in Niles, Illinois Seeks a Dynamic Social Services Coordinator. This is a Full-Time role with an 8-hour shift, and an Annual Salary of Between $60k and $65k, depending upon Experience.

Monday - Friday, 8:30am - 5:00 pm, with one Sunday rotation per month.

The Social Services Coordinator Collaborates with the Social Services Director

with Regard to the Following Responsibilities:

• Ensure that medical, emotional, and social needs of each resident is met/maintained
• Assist with planning, developing implementing our facility’s social service programs.
• Provide information regarding community resources to residents' families
• Greet and orient new residents
• Assist with discharge planning
• Attend regular care plan meetings with residents and their families
• Attend intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate

Requirements

of the Social Services Coordinator:

• Experience in the social services field in a skilled nursing or hospital environment
• Bachelor's or Master’s Degree in Social Work (MSW)
• Demonstrated knowledge of the social dynamic of the geriatric population
• Knowledge of government regulations as relevant to social services a plus
• Member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) a plus
• Knowledge of community resources helpful
• Initiative to seek new methods and services for residents
• Strong relationship building skills
• High level of organizational professionalism
