SOCIAL SERVICES DIRECTOR

Starting Salary with benefits: $55,000 - $65,000, based on credentials and experience in skilled nursing healthcare.

Responsibilities of the Social Services Director:

· Greet and orient new residents

· Provide information regarding community resources to residents' families

· Direct hospice and palliative care

· Coordinate discharge planning

· Facilitate regular care plan meetings with residents and their families

· Complete the MDS (Minimum Data Set) within the Assessment Reference date

· Address residents' grievances and assist nursing or other pertinent departments in resolving issues

IND1

· BA/BS or Master’s Degree in Social Work (MSW) or Licensed Clinical Social Worker

· A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW)

· Minimum two years' experience as a social worker in a skilled nursing or related setting

· A minimum of three (3) years of experience in a supervisory capacity in a hospital, nursing or long-term care facility

· Demonstrated knowledge of the social dynamic of the geriatric population

· Knowledge of community resources

· Strong leadership and relationship building skills

· Working knowledge of government regulations as relevant to social services
