Key Responsibilities:

• Advocate for Residents: Protect and support vulnerable residents, helping them find solutions to their challenges.
• Psychosocial Assessments: Evaluate patients' psychosocial status and communicate findings to the healthcare team.
• Person-Centered Interventions: Plan and implement personalized interventions based on social evaluations.
• Clinical Documentation: Maintain accurate records for all patients referred to social work.
• Referral Services: Provide information and coordinate services for residents and their families regarding practical and environmental needs.
• Community Liaison: Serve as the bridge between residents, families, and community agencies.
• Collaborative Care: Work closely with facility staff to support comprehensive resident care.
• Care Planning: Participate in developing care plans and case conferences for residents referred to social services.
• Behavior Monitoring: Establish interventions to ensure quality care and reduce medication use.
• Discharge Planning: Assist with discharge planning and address post-discharge needs.
• Medication Review: Coordinate medication reviews to ensure the lowest effective doses.

Requirements

Licenses/Certification

• LCSW, LSW, or MSW license preferred.
• Must have a valid driver’s license.

Experience

• Experience in post-acute/ skilled nursing preferred.
