Social Work Coordinator (PRN)

Community Health of Central Washington has an opening for a PRN Social Worker to provide case management to patients needing assistance in reducing barriers to care. You will work collaboratively with the RN Care Coordinator, Providers, and Care Teams; with focus on addressing social determinants of health and coordinate the integration of the social service function into patient care.

Qualifications:

• Bachelor’s degree in social work, social science, or a related field is required


• Master’s degree is preferred


• 2 years of recent case management experience is required or equivalent combination of education and experience will be considered


• BLS is required


• Bilingual Spanish/English is desired


• Immunizations are required prior to start date (COVID, Hep B (3), Tdap, MMR (2), Varicella (2), TB/PPD, FLU)


• Pre-employment drug screen is required

$29.33 - $38.01 hourly
