Job Description - Social Work Manager
For all patients requiring active social work services, ensures the provision of education relevant
to the psychosocial and resource aspects of care, and safe discharge/patient transition. Ensures
the provision of information to the health care team and patient/family/caregiver regarding
available resources and benefits for acute and post acute services that ensures patient choice
and a safe and timely transition.
7. Communicates and interprets regulations, standards, and policies to staff, patient, and physician.
8. Ensures adherence by staff to department, hospital and/or Joint Commission, CMS, and other
governmental regulations.
9. Provides input to the Director of Care Coordination for the development of the goals and
objectives of Care Coordination; participates in the development policies and procedures.
10. Assists the Director Care Coordination in developing and administrating the Care Coordination
budget. Prepares and submits the budget for ambulance and psychiatry providers to Department
Director. Provides oversight and monitoring performance of contracted services.
11. Reviews social work performance in relation to established goals; develops and implements plans
for corrective action in areas identified for improvement. Ensures performance is in compliance
with governmental and regulatory requirements.
12. Meets at regular intervals with Social Work staff. Participates in Department meetings and
committees as needed.
13. Responsible for personnel management and development; participates in professional
recruitment of staff, interview prospective candidates for employment, counsels, disciplines and
evaluates staff. Conducts staff performance appraisals on time in accordance with policy
timeframe.
14. Ensures orientation for new social work staff.
15. Ensures competency of social work staff. Maintains documentation of competencies in
accordance with Human Resources and regulatory policies.
16. Interacts with all Hospital departments to initiate, coordinate, and maintains optimal attention to
psychosocial needs of patients, progression of care, and discharge/transition planning.
17. Manages the operations of the Hospital's ambulance transportation contract. Coordinates as
needed with all inpatient areas.
18. Ensures Hospital wide coordination of perinatal bereavement services. Serves as a liaison with
area funeral directors.
19. Develops and coordinates internships for students from accredited schools of Social Work.
20. Participates in the Ethics and Utilization Review Committees and other committees as assigned.
21. Maintains collaborative relationships with behavioral health providers, community resources and
social services providers, and post-acute facilities.
22. Reports in a timely manner any significant changes in patients, incidents, personnel problems to
the Director.
23. Complies with all mandatory educational requirements of the organization; ensures professional
growth through continuing education and experience.
24. Performs related and other departmental duties, as requested.
25. Maintains liaison with community agencies and participate in related activities.
