Schedule: Monday to Friday | 8:00 AM – 4:30 PM | 40 hours per week

Job Summary:

We are hiring a Social Worker to support our Behavioral Health Unit in Boise, ID. This is a full-time contract role for 3 months. You’ll work with patients, families, and healthcare teams to support care planning, discharge coordination, and emotional support services.

Key Responsibilities:

• Help patients and families plan for discharge and next steps.

• Complete assessments, forms, and reports.

• Arrange transportation and follow-up care.

• Attend interdisciplinary team meetings.

• Provide information about Medicare/Medicaid and financial programs.

• Support patients and staff with social and emotional concerns.

• Maintain patient privacy and follow HIPAA rules.

• Track and report grievances.

• Participate in quality improvement efforts.

Qualifications:

• At least 3 years of experience as a Social Worker.

• Knowledge of healthcare discharge planning.

• Strong communication and organizational skills.

• Ability to work independently and with a team.
