Job Description - Social Worker

Affirmed Home has a NEW and EXCITING job opportunity! We are currently hiring a full-time Social Worker for our Manhattan office. The Social Worker is responsible for managing our clients/families and their personalized needs. Their duties include meeting with clients and their loved ones to discuss recent successes or setbacks to their health situations, developing treatment plans for clients and providing community resources when necessary.

Primary Responsibilities:

Assesses clients and/or family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills.

Serves as the primary point of contact for clients and families for all non-clinical care matters; conducts and directs meetings and calls with clients and their families.

Work with clients and families to evaluate their needs and connect them to appropriate services.

Facilitate home visits to ensure safe and supportive environments for clients and families.

Provides social work services in accordance with Affirmed Home Care policies, practices, procedures and Standards of Social Work Practice.

Participates in the development of treatment plans and revises the goals as needed. Coordinates approaches to patient and/or family care with other team members.

Consults with and educates the clients and families regarding the treatment plan and community resources.

Looks for opportunities to enrich and support the client in the home environment, to accommodate their success in the home, and support aging in place.

Requirements:

Master’s degree in social work.

Valid New York State LCSW or LMSW.

Minimum of one year of social work experience in a health care setting required, home care experience preferred.

Knowledge of developing individualized service plans, linking clients to community resources.

Ability to provide compassionate and effective patient care.

Excellent communication and interpersonal skills for building rapport with clients.

Strong organizational skills for managing case load and maintaining accurate records.
