Plaza Healthcare is a 179-bed CMS Rated 5-Star Skilled Nursing Facility located in South Scottsdale. Our mission is to provide exceptional care tailored to meet the unique needs of each patient. We specialize in high-acuity post-acute care, offering services that include tracheostomy and ventilator care, dialysis, complex wound care, and rehabilitation. Job Description

The Social Worker will play a critical role in providing support to residents and their families, helping them navigate the challenges of transitioning to and from care settings. This position involves conducting assessments, developing care plans, and providing resources to ensure a smooth discharge process.

Responsibilities:

• Conduct psychosocial assessments to identify residents' needs and develop appropriate care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive care and facilitate communication between healthcare providers, residents, and families.
• Assist residents and families in understanding discharge plans and accessing community resources.
• Provide support for emotional and social challenges faced by residents and their families.
• Maintain accurate documentation of assessments, interventions, and outcomes in accordance with regulatory guidelines.

Qualifications:

• Previous experience in a healthcare setting, preferably in skilled nursing or rehabilitation.
• Strong interpersonal skills and ability to communicate effectively with diverse populations.
• Knowledge of community resources and services available to residents and families.
