About the Role

• Work exclusively with the geriatric population

• Complete psychosocial assessments and care plans

• Join daily interdisciplinary team meetings

• Coordinate admissions and discharges

• Support and educate families on community resources

Qualifications

• 1+ year of social work experience with older adults

• Active LICSW or LCSW license

• Strong communication and teamwork skills

• Compassion for seniors and their families
