A well\-established hospital located near Marshall, Minnesota, is seeking a Social Worker to join their team. 

Starting at $30.00/hour

Social Worker Opportunity:

• Full\-Time, Permanent, Direct\-Hire
  


• Schedule: 8 am \- 4:30 pm, Flexible Schedule
  


• Reports to Hospital DON and Hospice Manager
  


• Open to New Grads

Social Worker Responsibilities:

• Be a core member of the Hospice team and provide support to patient's and family's emotional, social, and economic needs relating to the impact of a terminal illness
  


• Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, collaborate with doctors, nurses, and insurers
  


• Plan and Lead interdisciplinary meetings, facilitate the referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits
  

Social Worker Qualifications:

• Must have a Bachelor's degree in Social Work
  


• Must have a valid LSW or the eligibility to obtain one​
  


• Previous experience in Hospice is a bonus
