Job Description:
The Primary Children's Center for Safe and Healthy Families is an integrated medical and behavioral health clinic providing evidence-based treatments to children who have experienced trauma and/or abuse. In order to support families seeking behavioral health services related to abuse and trauma, the Pediatric Trauma Therapy Case Manager works as part of a multidisciplinary team at Safe and Healthy Families and collaborates with the patient's primary treatment team (therapists, psychologists, and psychiatrists) and with the patient's parents/caregivers in order to develop a case management plan that can address barriers to treatment progress. Families are referred for Case Management services when a child's primary treatment team has identified barriers to treatment related to Social Drivers of Health, educational difficulties, and/or a need for additional behavioral health supports for either the patient or patient's family members. The Pediatric Trauma Therapy Case Manager will work directly with patient's family members to develop a care management plan that can support a patient's progress in evidence-based therapy for trauma.Job Specifics
This position will include day-time shifts at Primary Children’s Center for Safe and Healthy Families at the Eccles Outpatient building in Salt Lake. The hours for this position will fall during clinic hours (Monday - Friday, 9-5), with options to negotiate exact schedule prior to hiring. The ideal candidate would work three partial days a week.
