A Brief Overview
Provide patients and their families, including vulnerable populations, with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care.

Hospice Social Worker:
• Educational Requirements: Has a Master of Social Work (MSW) degree from school of social work accredited by the Council of Social Work Education or has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW.
• Job Duties and Responsibilities: Timely development, revisions and monitoring of hospice social work plan of care, bereavement plans and discharges in addition to other required documentation. Documentation of collaboration and coordination of members of the interdisciplinary hospice team. Hospice social worker will adhere to CMS regulatory guidelines for compliance.
Palliative Care, West Burlington:
• Risk exposure to airborne pathogens is a level 1 category, meaning the job requires you to enter airborne isolation rooms (N95 mask fit).
