What You’ll Do:

• Assist social workers in conducting psychosocial assessments, care coordination, and discharge planning.
• Connect residents and families with community resources and support services.
• Support with documentation, case note preparation, and maintenance of resident care plans.
• Help plan and facilitate engaging group sessions and activities that support emotional and social well-being.
• Participate in interdisciplinary team meetings and contribute to resident-centered care strategies.
• Provide compassionate emotional support to residents and their families during care transitions.
• Assist with referral follow-ups and crisis response, under guidance.

• Degree in Social Work or a related field (BSW, MSW, or equivalent).
• Prior experience or interest in long-term care or rehabilitation settings is a plus.
• Organized, compassionate, and a clear communicator—both verbally and in writing.
• Comfortable managing multiple tasks in a fast-paced care environment.
• A team player who brings energy, empathy, and positivity to the workplace.
