Job Description - Social Worker Case Management

The role of the Social Worker Case Manager (SWCM) is to act as resource support for the Community, specifically the Nursing and Marketing Department. The role includes assisting the nursing team with administrative duties including but not limited to processing orders, referrals, contacting physicians or performing community outreach to medical providers, completing Medication Cart audits, scanning orders, scanning outside provider notes, updating the Electronic Health Record, auditing medical records, assisting with completion of 1823s and monitoring inventory of medications. Additionally, helping in training the Care Partner Team during orientation and throughout the year. The Clinical Care Specialist is also responsible for communicating with families to include but not limited to referrals, medication changes, and status updates. The SWCM assist in reaching out to family members prior to move-in to obtain additional records, plan for move-in and schedule the Initial Life Enrichment Plan. SWCM works in partnership with the Villas Social Worker in facilitating the Life Enrichment Plan to provide orientation to new Elders and families. SWCM also assist annually in the update of the Certified Emergency Management Plan and training for staff.

Duties:



  • Process medication orders

  • Follow up with physicians and medical providers for clarification on assessments, medication orders

  • Assist with completing referrals to outside medical providers

  • Provide Community Outreach to Medical Providers

  • Assist with processing medication refills and new orders in Electronic Health Record

  • Collaborate with contracted pharmacy regarding clarifying medication orders, processing refills

  • Complete medication cart audits and monitoring medication inventory

  • Coordinate with families prior to move in to obtain all required documentation

  • Communicate with families to discuss status changes, medication updates and referrals

  • Coordinate with provider referrals for potential move ins

  • Facilitate in collaboration with the Villas Social Worker the initial Life Enrichment Plan

  • Assist with updating CEMP and annual staff training

  • Perform other duties as requested

  • Assist with monitoring compliance of regulatory requirements

  • Assist with training Care Partner Team


Qualifications:



  • Must complete a 6 Hour Medication Assistance Course prior to employment or take course as soon as possible after initial employment (course provided by employer).

  • Experience in Assisted Living and/or Gerontology Field

  • Experience in Case Management preferred

  • Degree or Certification in Health Sciences, Social Work or related field

  • Upon hire required Villas at Killearn Lakes Training & Certifications and State Requirements
