The role of the Social Worker Case Manager (SWCM) is to act as resource support for the Community, specifically the Nursing and Marketing Department. The role includes assisting the nursing team with administrative duties including but not limited to processing orders, referrals, contacting physicians or performing community outreach to medical providers, completing Medication Cart audits, scanning orders, scanning outside provider notes, updating the Electronic Health Record, auditing medical records, assisting with completion of 1823s and monitoring inventory of medications. Additionally, helping in training the Care Partner Team during orientation and throughout the year. The Clinical Care Specialist is also responsible for communicating with families to include but not limited to referrals, medication changes, and status updates. The SWCM assist in reaching out to family members prior to move-in to obtain additional records, plan for move-in and schedule the Initial Life Enrichment Plan. SWCM works in partnership with the Villas Social Worker in facilitating the Life Enrichment Plan to provide orientation to new Elders and families. SWCM also assist annually in the update of the Certified Emergency Management Plan and training for staff.
