Social Worker (Full-Time) Long Island, NY

Responsibilities:

• Provide social work services to patients in the field
• Conduct assessments and develop care plans
• Collaborate with interdisciplinary teams to support patient care
• Maintain accurate and timely documentation
• Ensure compliance with all regulatory and company standards

Qualifications:

• Active Social Worker license (as applicable)
• Experience in TBI (Traumatic Brain Injury) care preferred
• Strong interpersonal and communication skills
• Ability to work independently and manage field-based responsibilities

Compensation:

• Salary: $70,000–$74,000 annually (based on experience)

Schedule:

• Monday–Friday, 9:00 AM–5:00 PM

Additional Information:

• Field work required
