Social Worker Home Health (MSW)

Location: Rock Springs, WY

Pay: MSW $33+/hour DOE, LCSW $30+/hour DOE

Job Overview:

We are seeking a compassionate and dedicated social worker to provide support to the geriatric community. The ideal candidate will have a strong background in providing end of life support, offering community resources, and discussing care goals with patients. You are paid for drive time and mileage!

Responsibilities:

• Communicate effectively with the internal team and those offering patient support
• Organize schedule in an efficient manner to travel to assigned visits
• Provide helpful resources and discuss care goals with elderly patients
• Navigate end of life issues that require support and attention
• Offer crisis management support during difficult situations

Qualifications:

• MSW required or LCSW
• Valid driver's license
• Experience with geriatric community preferred, not required
