Baptist Health is seeking a compassionate and motivated Social Worker II to join the team at Wolfson Children’s Hospital. This Full-Time Days position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources.

Social Worker II, Job Responsibilities:

Assess patients’ psychosocial needs to support emotional well-being and optimal functioning.
Provide discharge planning, including coordination of home health services and durable medical equipment (DME).
Assist with patient placement and transfers, ensuring smooth transitions of care.
Offer referrals to community resources to support ongoing care and family needs.
Support utilization review activities to promote effective and appropriate care delivery.

Experience Requirements:

1-2 years Counseling Experience Required
1-2 years Experience in Child Welfare Services Required
1-2 years Social Work Experience Required
1-2 years Hospital Experience Required
Less than 1 year Experience in Behavior Health Setting Required

Education & Credential Requirements:

Master of Social Work required
Licensed Clinical Social Worker Preferred
Driver's License Preferred
