A leading healthcare facility in Northern California is seeking a compassionate and experienced Social Worker to join their case management team. This role plays a key part in supporting patients and families with social assessments, discharge planning, and community referrals from admission to discharge.

What You’ll Do

• Complete comprehensive psychosocial assessments for patients of all ages
• Collaborate with physicians, nurses, and allied health to ensure safe discharge plans
• Connect patients and families with appropriate community services and support
• Handle complex discharge needs with a multidisciplinary team
• Document care plans and progress in the electronic medical record system
  
  

Qualifications

• Bachelor’s Degree in Social Work (BSW) + 2 years of experience in acute care or skilled nursing
  OR
  
• Master’s Degree in Social Work (MSW) + 1 year of hospital social work experience
• Broad experience in discharge planning or healthcare social services
• Preferred: LCSW license or eligibility upon hire
• Strong interpersonal and problem-solving skills
