JOB SUMMARY

The Medical Social Worker (MSW) or Licensed Clinical Social Worker (LCSW) provides social work services to patients on an intermittent basis in their place of residence. This care is delivered according to physician orders and a personalized plan of care under the supervision of the Director of Clinical Services or Clinical Manager. Services may be provided by a qualified social worker or by a social work assistant under appropriate supervision.

RESPONSIBILITIES

• Adheres to established agency policies and procedures.
• Promotes and understands the principles of Quality Assessment and Performance Improvement (QAPI).
• Acts as a consultant to agency personnel, providing expertise and guidance as needed.
• Coordinates agency services with community resources and effectively utilizes available community support.
• Safeguards patient information, maintaining strict confidentiality.
• Serves as a resource for employees, patients, physicians, and other healthcare providers.
• Establishes cooperative relationships and communicates professionally with physicians.
• Maintains regular attendance and punctuality, ensuring reliability in the role.
• Keeps current knowledge of Medicare Conditions of Participation relevant to social work services.
• Reports any accidents, incidents, lost items, or unusual occurrences promptly to the Director of Clinical Services or Clinical Manager.
• Participates in in-service and continuing education programs to enhance professional skills.
• Attends patient care conferences to discuss care strategies and coordinate services.
• Maintains contact with community support groups and provides professional expertise as needed.
• Plans and coordinates all social services provided within the agency.
• Accurately documents all patient and family services as required by agency policy.
• Assists physicians and other team members in understanding the social and emotional factors related to health problems.
• Contributes to the development of each patient’s plan of care.
• Prepares clinical and progress notes with accuracy and timeliness.
• Provides social work support to patients’ families.
• Participates in discharge planning to ensure a smooth transition for patients.
• Performs additional duties as assigned by the Director of Clinical Services or Clinical Manager. Social Work Assistants may be assigned tasks by the supervising Social Worker.

QUALIFICATIONS

• Master’s or doctoral degree from a social work program accredited by the Council on Social Work Education, with at least one year of social work experience in a healthcare setting, or
• Bachelor’s degree in social work, psychology, sociology, or related field, with at least one year of social work experience in a healthcare setting (functions as an assistant).
• Two (2) years of relevant experience required for assistants.
• One (1) year of home care experience preferred.
• Commitment to the philosophy and goals of the agency.
• Ability to exercise initiative and make independent judgments.
• Strong communication and interpersonal skills to work effectively with individuals and enlist the cooperation of various stakeholders.

WORKING ENVIRONMENT

Primarily works indoors in an office setting and patient homes, with travel required to and from patient residences.

RISK EXPOSURE

High risk due to potential exposure to infectious diseases and regular interactions in patient care settings.

LIFTING REQUIREMENTS

The role requires the ability to engage in physical activities essential for patient care, including regular bending, lifting, standing, and the ability to work for extended periods while sitting or standing, with occasional light lifting as needed.
