SOCIAL WORKER OR HUMAN SERVICES PROFESSIONAL III & IV-KAUAI

Job Summary

Provide professional social work and human services support to individuals and families by assessing needs, developing and implementing service plans, coordinating resources, and monitoring client progress. Work with clients experiencing psychosocial, behavioral, mental health, environmental, and family-related challenges to improve their quality of life and ability to function effectively within their communities. Establish collaborative relationships with clients, service providers, community agencies, and other professionals to ensure appropriate support services are delivered. Evaluate outcomes, adjust service plans as needed, and maintain accurate case documentation while adhering to applicable policies, procedures, and professional standards.

Qualifications:

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, Counseling, Behavioral Sciences, or a related field.
• Professional experience providing case management, counseling, social services, or human services support.
• Knowledge of social work principles, human behavior, community resources, and case management practices.
• Ability to assess client needs, develop service plans, coordinate services, and monitor progress.
• Strong communication, interpersonal, problem-solving, and documentation skills.
• Ability to work effectively with individuals, families, community organizations, and multidisciplinary teams.
