About the Role: Deliver Professional Social Work Services

As a Social Worker, you will assess patient and family needs, create and implement individualized care plans, and connect people to resources and support systems. You will play a critical role in advocating for individuals, fostering independence, addressing barriers to care, and collaborating as part of an interdisciplinary team.

Key Responsibilities: Provide Exceptional Case Management & Support

• Conduct biopsychosocial assessments to understand individuals’ needs, strengths, and challenges.
• Develop and implement care, support, and discharge plans in partnership with patients, families, and other care providers.
• Facilitate access to community resources, referrals, and support services including housing, financial, mental health, and education programs.
• Offer counseling, crisis intervention, and short-term therapies as appropriate to support emotional and social well-being.
• Advocate for client rights and coordinate with healthcare, legal, and community partners to maximize quality of care and protect vulnerable populations.
• Educate individuals and families on coping strategies, available services, and navigating complex systems.
• Document activities and maintain accurate records in alignment with organizational and regulatory requirements.

Qualifications: Social Work Skills, Compassion & Professionalism

• Master’s degree or higher from an accredited program required.
•  1+ years of experience in ANY of Family Practice, Pediatrics or Adolescent Care
• Active State Social Work License (LMSW, LCSW, or equivalent as required by state regulations). ANY of the following:
• Licensed Professional Counselor (LPC) in Michigan (MI)
• Limited Licensed Master Social Worker (LLMSW) in Michigan (MI)
• Limited License Psychologist (LLP) in Michigan (MI)
• Temporary Limited License Psychologist (TLLP) in Michigan (MI)
• Licensed Master Social Worker (LMSW) in Michigan (MI)
• Licensed Clinical Social Worker (LCSW) in Michigan (MI)
