Summary
The Social Worker- Service Coordinator assists, enables, and supports an infant or toddler with a disability and the child’s family to receive the services and rights under federal law. This includes conducting family assessments, coordinating and participating in developmental evaluations, overseeing and providing services on the child’s Individualized Family Service Plan, and coordinating transition activities. Evaluates child’s living conditions, conducts social or emotional developmental assessments, works with problems in the living situation, and identifies and coordinates community resources and services that affect utilization of early intervention services. Provides appropriate skill-building with the infant or toddler and the parent.
Experience Requirements (Skills, Knowledge, and Abilities)
Requires:
1. Minimum level of one year of related work experience working with community resources, local education agencies, and conducting home visits / working with families and children.
2. Intermediate skill level in Microsoft Office applications (i.e., word processing software), email applications, and internet usage.
3. Ability to maintain confidentiality and use appropriate judgment in handling information and records.
4. Well-developed analytical and problem-solving abilities.
5. High degree of initiative and independent judgment.
6. Ability to organize work, engage in a variety of tasks simultaneously, and consistently meet deadlines.
7. Ability to demonstrate a high attention to detail and good follow-up skills. Must be self-motivated, proactive, and solution-focused.
8. Ability to establish and maintain a comprehensive record-keeping system and office procedures.
9. Ability to express self effectively and concisely, both orally and in writing.
10. Ability to tactfully and effectively deal with the public, staff, and patrons.
11. Ability to work independently with general direction.
12. Experience with Early Track, Brittco, and Adobe Acrobat Pro preferred.
Education/License Requirements
Appropriate degree in social work or related field (bachelor's, master's or doctoral).
Valid Ohio Driver’s License and appropriate insurance coverage.
Licensed Social Worker (LSW) required.
Must meet the requirements to become a credentialed Early Intervention Service Coordinator.
