Specialty Nurse - Palliative Care

Nemours is seeking Specialty Nurse, Palliative Care, to join our Nemours Children's Health team in Wilmington, DE.  This is a 20 hour per week position.

The Palliative Care Specialty Nurse Specialty Nurse will be a part-time member of a growing Palliative Care team establishing a comprehensive outpatient program with the plan to grow this into a home care service. Candidates should have excellent time management skills, be a proactive worker, work well in a team environment, and possess the desire to engage in patient care.

Nursing responsibilities:

• Field/triage calls and messages from the phone line, outpatient providers and patients and families

• Call patients and families to report lab results and provide check-ins

• Facilitate prior authorizations and prescription management

• Facilitate with flow of clinic

• Interface with hospice, coordinate admissions, provide ongoing communication

• Facilitate scheduling

• Data collection and tracking to measure the progress of the program and articulate additional need

• Support for discharge planning and inpatient follow-up

Essential Functions

Requirements

BSN

AHA BLS

One to three years of experience required

Pediatric nursing experience required; at least 3 years of related experience preferred
