The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages and manages primary care patients with medical and behavioral health problems. This includes providing skill training through psycho-education and patient education strategies and developing specific behavioral/therapeutic treatment plans for patients. The BHC also assists primary health care providers in recognizing and treating mental disorders and psychosocial problems, detecting "at risk" patients, and assessing the clinical status of patients referred by the primary care provider. 

Responsibilities: 

• Evaluate the clinical status of patients referred by primary care providers and develop a treatment plan accordingly.
• Conduct evaluations determining presence of DSM diagnosis, develop interventions and ongoing plan for patients interested in engaging in ongoing psychotherapy. 
• Work with primary care team and/or psychiatry to treat and manage patients with chronic medical and psychiatric, respectively, conditions efficiently and effectively.
• Assist with reducing stigma by helping to increase patient and medical team’s comfort with behavioral health conditions and associated treatment.
• Effectively identify signs and symptoms of “at risk” patients and develop of plans to prevent urther psychological or physical self-harm or deterioration.
• Evaluate patient care plans and share/discuss with primary car/ psychiatry when indicated.
•  Educate patients, families, and staff about care, prevention, and integrated treatment enhancingtechniques.
• Complete timely and appropriate documentation in the Electronic Health Record (EHR) system, including but not limited to, internal communications, release of records requests, progress notes, evaluation notes and mandated reporting documents.
• Follow established guidelines for use and/or disclosure of protected health information (PHI).
• Refer and bridge care for patients that are not appropriate for and/or cannot be appropriatelymanaged in the primary care setting. 
• Participate in group medical consultation for primary care topics such as smoking cessation, obesity, chronic pain management, Diabetes as well as behavioral health topics such as anxiety, depression, suicide prevention, adverse childhood experiences, etc.
• Help develop and participate in needed health promotion programs.
• Serve as a preceptor to paraprofessional staff placed with TRUECARE, as needed.
•  Attend and participate in departmental and organizational meetings, as required.
• Identify problems related to patient services and make recommendations for improvement.
• Participate in behavioral health quality improvement activities.
• Attend continuing education programs and provide evidence of necessary education completion to upgrade medical skills and maintain licensing requirements.

Qualifications:

• Master’s degree in Clinical Social Work
• Current Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences  
•  At least two (2) years’ experience providing psychotherapy (CBT based models preferred) with children, adolescents, adults and other underserved populations.
• Computer proficiency, particularly with Electronic Health/Medical Record systems and the
• Microsoft Office suite, including Outlook, Word, Excel and PowerPoint. 

•  Bilingual in English and Spanish (may be required at some clinics).
• Experience providing health psychology services such as smoking cessation, weight management, stress management, substance abuse disorders, or work with chronically medically ill patients preferred.
• A general understanding of trauma-informed care approaches and therapies, Adverse Childhood Experiences and their impact on brain development, suicide prevention and acute crisis assessment/intervention.
• Familiarity or work experience in integrated behavioral health 
  models.
