Job Description - Speech & Language Pathologist

Description

The SLP in a hospice setting is responsible for assessing and treating patients who have swallowing or communication disorders. The primary goals of the therapist are to help patients communicate effectively, improve patients’ comfort and quality of life related to eating and drinking, and to educate caregivers on safe diets and feeding strategies. To be successful in this role, candidates should have excellent communication and interpersonal skills, as well as strong clinical knowledge in the areas of speech-language pathology and dysphagia management. They should also be comfortable working in a home environment. 

ESSENTIAL FUNCTIONS:

  • Conduct clinical assessments to evaluate patients' communication and swallowing abilities.
  • Develop treatment plans based on the assessment results.
  • Provide therapy to improve speech, language, and swallowing function.
  • Educate patients and their families about communication and swallowing strategies.
  • Collaborate with other healthcare professionals to provide coordinated care.
  • Document patient progress and maintain accurate records.
  • Participate in interdisciplinary meetings to review patient care plans and make recommendations as needed.
  • Provide emotional support to patients and their families during end-of-life care.
  • Provide home visit within 7 days of receipt of referral.
  • Participate in educational programs that enhance professional growth and development as determined by the agency.
  • Work with patients in a non-judgmental fashion 

COMPETENCIES

  • Computer Competency and evidence of effective communication skills using verbal, written, and telephonic means.
  • Ability to make decisions regarding the priority of work. 
  • Strong interpersonal skills.
  • Ability to understand specific instructions. 

WORK ENVIRONMENT/PHYSICAL DEMANDS

The physical demands described here are representative of those that an employee must meet to successfully perform the essential functions of this job. 

  • Ability to travel in assigned service area. 
  • Physical agility with repetitive occurrence including but not limited to walking, running, bending, stooping, climbing stairs, kneeling, crouching, crawling, balancing, reaching, standing, sitting, and twisting. 
  • Lifting greater than 30 pounds repeatedly throughout the workday. 
  • Valid driver’s license with reliable transportation and required auto liability insurance.

PREFERRED EDUCATION & EXPERIENCE

  • Current NYS License to practice as a Speech-Language Pathologist
  • Minimum of two years experience in a health facility or Homecare Setting
  • M.A. or M.S. in Speech-Language Pathology

 

Hours: Per Diem - number of hours dependent on department needs

Pay: $60-62 per hour

FLSA: Non-Exempt
