Job Title: Speech Language Pathologist
Schedule:Monday-Friday Employment Type: Preferably Full-Time or Part-Time may be available
Job Summary Partners In Home Care is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Home Health team. In this role, you’ll provide speech therapy services to patients in their homes, helping them improve communication, cognitive-linguistic skills, and swallowing function. As part of a multidisciplinary care team, you’ll also play a vital role in educating patients and families while ensuring high-quality, patient-centered care.
Key Responsibilities
Evaluate patients with speech, language, voice, cognitive-linguistic, and swallowing disorders; develop individualized plans of care.
Provide direct patient care according to physician orders, adjusting treatment as needed based on patient progress.
Complete required OASIS assessments and thorough documentation at point of care per CMS and agency guidelines.
Communicate effectively and professionally with physicians, patients, families, and team members.
Participate in case conferences and collaborate with the care team to support patients with complex needs.
Supervise Home Health CNAs when no RN, PT, or OT is active in the plan of care.
Train skilled and non-skilled personnel in rehabilitative procedures as appropriate.
Manage daily schedule, coordinate with schedulers, and maximize efficiency of care delivery and travel routes.
Contribute to quality improvement and compliance initiatives; play an active role in achieving agency benchmarks.
Utilize community resources and make appropriate referrals when needed.
Perform other duties as assigned to support patient care and team success.
Qualifications
Master’s degree (or higher) in Speech-Language Pathology from an accredited program.
Current license to practice as an SLP in the State of Montana.
Valid driver’s license and auto insurance (required for home visits).
At least two years of SLP experience preferred; home health experience a plus.
Strong communication skills and ability to work collaboratively with a multidisciplinary team.
