Job Title: Speech Language Pathologist


Schedule: Monday-Friday
Employment Type: Preferably Full-Time or Part-Time may be available



Job Summary
Partners In Home Care is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Home Health team. In this role, you’ll provide speech therapy services to patients in their homes, helping them improve communication, cognitive-linguistic skills, and swallowing function. As part of a multidisciplinary care team, you’ll also play a vital role in educating patients and families while ensuring high-quality, patient-centered care.



Key Responsibilities



  • Evaluate patients with speech, language, voice, cognitive-linguistic, and swallowing disorders; develop individualized plans of care.

  • Provide direct patient care according to physician orders, adjusting treatment as needed based on patient progress.

  • Complete required OASIS assessments and thorough documentation at point of care per CMS and agency guidelines.

  • Communicate effectively and professionally with physicians, patients, families, and team members.

  • Participate in case conferences and collaborate with the care team to support patients with complex needs.

  • Supervise Home Health CNAs when no RN, PT, or OT is active in the plan of care.

  • Train skilled and non-skilled personnel in rehabilitative procedures as appropriate.

  • Manage daily schedule, coordinate with schedulers, and maximize efficiency of care delivery and travel routes.

  • Contribute to quality improvement and compliance initiatives; play an active role in achieving agency benchmarks.

  • Utilize community resources and make appropriate referrals when needed.

  • Perform other duties as assigned to support patient care and team success.



Qualifications



  • Master’s degree (or higher) in Speech-Language Pathology from an accredited program.

  • Current license to practice as an SLP in the State of Montana.

  • Valid driver’s license and auto insurance (required for home visits).

  • At least two years of SLP experience preferred; home health experience a plus.

  • Strong communication skills and ability to work collaboratively with a multidisciplinary team.
