What You’ll Do:

• Provide individualized speech, language, and swallowing therapy in clients’ homes
  
• Conduct diagnostic assessments and create tailored treatment plans
  
• Offer functional communication and cognitive -linguistic training
  
• Educate clients and caregivers on strategies that reinforce progress and safety
  
• Document services using EMR and communicate with the care team as needed
  

Qualifications:

• Current Minnesota Speech Language Pathologist (SLP) license
  
• Valid driver’s license and reliable transportation
  
• Experience with EMR preferred
  
• Passion for patient -centered, one -on -one home care
